الأنسولين Insulin
البنكرياس أحد الغدد الصماء توجد بجوار الإثني عشر ( الجزء الأول من الأمعاء )، وتتكون من مجموعة من الخلايا كلاً منها مسئول عن إفراز بعض الهرمونات أو الإنزيمات، وتقوم خلايا معينة تسمى Beta cells بإنتاج هرمون الأنسولينInsulin ، والذي يتم إنتاجه عند حدوث ارتفاع في نسبة السكر في الدم، وينخفض الإنتاج عند انخفاضه، أي أن مستوى السكر في الدم هم الذي يقوم بتنشيط وحث خلايا البنكرياس على الإنتاج والإفراز.
ما هو عمل الأنسولين ؟
بالتعاون مع العديد من الإنزيمات والهرمونات والتي تعمل بطريقة معقدة ومتتالية، فكلاً منها له دوره، ومن أهم ما يقوم به الأنسولين هو التحكم في تحرك السكر في الدم وإدخاله إلى خلايا الجسم ومن ثم ضبط مستواه في حالتي الأكل والصيام.
كيف يعمل الأنسولين ؟
بعد القيام بأكل وجبة من الغذاء فإن الجهاز الهضمي وملحقاته ( المعدة ، الأمعاء، الكبد ) يقوم بهضم تلك المواد، وعملية الهضم هي تكسير الغذاء إلى مكوناته الأصلية والأساسية، فمثلاً :
o المواد النشوية ( الكربوهيدرات ) تتكسر إلى الجزيء الأصغر وهو السكريات وأساسها الجلوكوز
o المواد البروتينية تتكسر إلى موادها الأساسية وهي الأحماض الأمينية Amino Acid
o المواد الدهنية تتكسر إلى الأحماض الدهنية وبعد ذلك لأجزائها الأصغر وهي الكيتونات
تلك المواد الأساسية ( السكر، الأحماض الأمينية، الكيتونات ) تمتص من خلال جدار الأمعاء إلى مجرى الدم ( الأوعية الدموية )،
حيث ترتفع نسبة السكر والأحماض الأمينية وغيرها من المواد التي تم امتصاصها.
في الحالات الطبيعية فإنه بعد تناول الوجبة الغذائية ( بعد عشر دقائق تقريباً ) يرتفع مستوى السكر في الدم، هذا الارتفاع يعطي إشارة لخلايا البنكرياس لحثها على إنتاج وإفراز الأنسولين، هذا الأنسولين يصل إلى مجرى الدم، ليصل بعد ذلك إلى جميع أجزاء الجسم.
يقوم الأنسولين بحث الخلايا على السماح بدخول السكر داخلها، وخصوصاً خلايا الكبد والعضلات ليتم تخزينه حتى وقت الاحتياج له، وبذلك ينخفض مستوى السكر في الدم إلى مستوى مقارب للمستوى قبل تناول الوجبة، وهنا يتوقف البنكرياس عن إنتاج الأنسولين أو يقل، وذلك بعد أربع ساعات من تناول الوجبة، وتتكرر العملية مع كل وجبة غذائية.
ملاحظة : خلايا الجهاز العصبي والمخ لا تعتمد على الأنسولين في القيام بنشاطها الحيوي.
أنواع الأنسولين
الأنسولين له عدة أنواع معتمدة على المصدر المستخرج منه ( ما يستخدم في البلاد الإسلامية من الإنسان أو من البقر )،
وهناك أنواع مصنعة من البكتيريا ، كما له عدة أنواع ومسميات معتمدة على مدة مفعوله ومنها :
1. الأنسولين العادي Regular ( R)
الأنسولين قصير المدى Short- acting Insulin
o هو ما يسمى بالأنسولين الصافي
o يعتبر الأنسولين الطبيعي بدون إضافات تزيد من مدة مفعوله
o يتم امتصاصه بشكل سريع
o يستمر مفعوله لمدة 4-12 ساعة
o مدة المفعول تعتمد على الجرعة، فكلما زادت الجرعة زادت مدة المفعول نسبياً، كما أن الجرعة العالية تؤثر على مستوى السكر في الدم
أنواعه :
o أكترابد – Actrapid: مصدره الانسان، يبدأ عمله بعد20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 3-4 ساعات، وستمر مفعوله لمدة 6-10 ساعات.
o هايبيورين نيوترال Hypurin neutral: مصدره البقر، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 3-4 ساعات، وستمر مفعوله لمدة 6-10 ساعات.
o فيلوسيولين Velosulin:مصدره الخنزير، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 2-3 ساعات، وستمر مفعوله لمدة 6-10 ساعات ، غير متوفر في الخليج.
o هيوميولين أس Humulin-s: مصدره الانسان، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 2-3 ساعات، وستمر مفعوله لمدة 6-10 ساعات.
o هيومالوج Humalog: مصدره الانسان، يبدأ عمله بعد 5 دقائق من تناول الجرعة، ويصل إلى الذرة بعد 1-2ساعات، وستمر مفعوله لمدة 5-6 ساعات.
2. الأنسلين متوسط المدى Intermediate-acting Insulin
o يعمل بشكل سريع ومدة عمله قصيرة جداً
o يتم أخذه قبل تناول الغذاء مباشرة ، وعليه فنادراً ما يؤدي إلى حدوث نوبات انخفاض السكر
من أنواعه:
o هيوميولين م 1-م5 Humulin M1-M5: مصدره الانسان، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 2-8ساعات، وستمر مفعوله لمدة 20-24 ساعة.
o المخلوط Mixtard 10,20,30,40,50: مصدره الانسان- الخنزير، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 4-6 ساعات، وستمر مفعوله لمدة 20-24 ساعة.
o أنسلوتارد Insulatard : مصدره الانسان، يبدأ عمله بعد 60-120دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 5-8 ساعات، وستمر مفعوله لمدة 20-24ساعة.
o هيوميولين -1 Humulin-1: مصدره الانسان، يبدأ عمله بعد 60-120 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد6-8 ساعات، وستمر مفعوله لمدة 18-24 ساعة.
o هيبيورين ايزوفين Hypurin Isophane مصدره البقر والخنزير، يبدأ عمله بعد 60-120 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 6-8 ساعات، وستمر مفعوله لمدة 18-24 ساعة.
o هيوميولين لينتي Humulin Lente : مصدره الانسان، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 8-12 ساعات، وستمر مفعوله لمدة 22-30 ساعة.
o هيبيورين Hypurin Lente : مصدره البقر، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 8-10 ساعات، وستمر مفعوله لمدة 22-30 ساعة.
o لينتارد أم سي Lentard MC : مصدره البقر الخنزير، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 8-12 ساعات، وستمر مفعوله لمدة 22-26 ساعة.
o هيومان مونوتارد Human Monotard : مصدره الانسان، يبدأ عمله بعد 20-30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 8-11 ساعات، وستمر مفعوله لمدة 20-24 ساعة.
o هيبيورين بايفيزك Hypurin Biphasic : مصدره الخنزير، يبدأ عمله بعد 30 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 4-12 ساعات، وستمر مفعوله لمدة 20-24 ساعة.
3. الأنسولين طويل المفعول ( طويل المدى ) NPH :
o يسمى بالأنسولين العكر - (يمكن إطالة مدة مفعول الأنسولين بخلطه مع بروتينات معينة لتصل لمدة تتراوح بين 12-36 ساعة، وهو ما يجعله عكراً وغير صافي )
o بعد حقنه تحت الجلد فإنه يمتص ببطء، ويستمر مفعوله لمدة أربع وعشرين ساعة تقريباً
o هناك نوع جديد يسمى Glargine يتميز بقلة حدوث نوبات انخفاض السكر في الدم
من أنواعه:
o Humulin ZN مصدره الانسان، يبدأ عمله بعد 120-240 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 10-14 ساعات، وستمر مفعوله لمدة 24-30 ساعة.
o Hypurin Protamine Zinc - Beef مصدره البقر، يبدأ عمله بعد 120-240 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 14-16 ساعات، وستمر مفعوله لمدة 28-34 ساعة.
o Ultratard مصدره الانسان، يبدأ عمله بعد 120-240 دقيقة من تناول الجرعة، ويصل إلى الذرة بعد 10-18 ساعات، وستمر مفعوله لمدة 28-34 ساعة.
4. الأنسلين ذي المرحلتين Biphasic :
o هو خليط من النوعين قصير المدى والطويل المدى، وله نتائج جيدة في ضبط مستوى السكر في الدم
الطرق المستخدمة لأخذ الأنسولين
هناك عدة طرق لأخذ الأنسولين ، من أهمها :
o عن طريق حقن تحت الجلد :
وهي الطريقة المتبعة بشكل واسع في كل أنحاء العالم
o مضخة الأنسولين :
وهي جهاز صغير يعمل بالبطارية ، يوجد به مستودع يوضع فيه الأنسولين، موصل بأنبوب صغير في نهايته حقنة موضوعة تحت الجلد، يتم برمجة الجهاز لكي يقوم بضخ كمية من الدواء ( كمية صغيرة ) وبشكل مستمر، وعادة يربط الجهاز على الحزام، وتوضع الحقنة تحت جلد البطن، وبعض الأطفال يستخدمونه ليلاً فقط، والبعض طوال الوقت، وفائدة هذا الجهاز أنه يعطي كمية صغيرة من الدواء لوقت مستمر وبذلك يمنع نوبات هبوط السكر ، كما يمكن عن طريقه التحكم أكثر في مستوى السكر في الدم.
o عن طريق الاستنشاق :
أثبتت التجارب أنه يمكن تناول الأنسولين عن طريق الاستنشاق، وأن كمية امتصاص الدواء جيدة، كما يمكن التحكم في الجرعة عن طريق جهاز البخ المعياري والذي يحتوي على كمية محددة من الدواء في كل بخة، وقد بدأ استخدامه في بعض البلاد.
لماذا يؤخذ الأنسولين تحت الجلد ؟
منطقة تحت الجلد تحتوي على نسبة عالية من الخلايا الدهنية، كما أن عدد الأوعية الدموية قليل وحجمها صغير، لذلك فإن امتصاص المواد ( والأدوية ) الموجودة تحت الجلد يتم ببطء شديد وعلى مدى ساعات، وبذلك تقل عدد الجرعات ويمكن التحكم بطريقة أسهل.
أمّا إذا أعطي الأنسولين عن طريق الوريد مثلاً ( وهو ما قد يستخدم في المستشفى في بعض الحالات ) فإن الامتصاص والتأثير يكون سريعاً جداً والمفعول لمدة قصيرة جداً، وإذا أعطي الأنسولين عن طريق الحقن في العضل فيكون الامتصاص سريع نسبياً
ما هي أفضل الطرق لأخذ الأنسولين ؟
المطلوب هو أخذ العلاج بأقل عدد من المرات في اليوم الواحد، وليكون مستوى الأنسولين ثابتاً ( تقريباً ) طوال اليوم، ويتم ذلك عن طريق أخذ حقن الأنسولين تحت الجلد ، كما ينصح باستخدام مضخة الأنسولين
